Parents, if your child(ren) needs medication administered at the school, you must complete a Medication Consent Form. This form must be completed for both prescription and non-prescription medication. A doctor's signature is required for all prescription medication.
If your child(ren) use an inhaler, he/she may keep it in his/her backpack, but an Asthma Health Plan Form and Medication Consent Form must be filled out.
If your child(ren) have an allergy that the school should be aware of, then the Emergency Care Plan Sheet and Medication Consent Form must be filled out.
